T031 Option B branding validation
Homepage and account page loaded with the two-row COSMOS expansion and cyan initials. Desktop screenshot reviewed; no horizontal homepage overflow. Responsive font sizing and narrow-screen spacing included; physical phone/tablet visual check remains available. Shared gateway PHP syntax and existing Supernova/Polaris inline-script CSP hashes passed. Existing application URLs preserved.
T030 Behavior COSMOS branding checks
Visible homepage and account check plus hosting-source validation
- Homepage heading, navigation wordmark, footer and page title use Behavior COSMOS. Desktop screenshot inspected.
- Account page heading and browser title use Behavior COSMOS. Corrected duplicated title wording during verification.
- Shared app toolbar source and Supernova byline use the new name. App names and existing working domain links remain intact.
- Supernova and Polaris inline script CSP hashes validated; no account, record or encryption configuration changed.
- New domain remains a forwarding domain pending registrar activation; this update changes branding rather than migrating account or device storage origins.
T029 New domain redirect configuration
Hosting configuration and public reachability
- cPanel confirmed behaviorcosmos.com was created with its own document root.
- Redirects interface confirmed the root forwards to https://cosmosbehavior.com/. Permanent 301, with-or-without-www, wildcard path preservation selected.
- Mochahost registrar status remained Awaiting Activation after contact setup. Public address test returned a connection failure before the redirect could be verified.
- DNS delegation, HTTPS certificate and end-to-end public forwarding remain pending activation. Existing COSMOS site and application data were not relocated.
T028 Session loading and saved-record retest
R029 isolated startup tests and authenticated live navigation
- Corrected the earlier session-loss interpretation: the account page initially displayed login while its asynchronous existing-session check was still running. The settled page restored the authenticated dashboard.
- R029 hides credential panels during that check and displays Checking sign-in. Tabs remain disabled until completion; actual anonymous and service-error states are handled separately.
- Isolated tests passed for pending state, successful dashboard restoration, anonymous response, service failure, verification page and restored controls.
- Live return to account showed Checking sign-in, then the authenticated dashboard without entering credentials again.
- Polaris reload passed. Saved Records reopened the completed fictional scenario; original intervention and follow-up notes remained in the full report.
- Live Polaris to Nebula to Supernova to Account navigation retained the same authenticated session. No Supernova vault was created or changed.
- Cookie settings, idle and absolute expiry, CSRF validation and account permissions were unchanged. Earlier T026/T027 session-continuity concerns are resolved by this settled-state retest.
T027 Polaris fictional scenario and report checks
Authenticated browser workflow after user agreement review
- Completed a task-refusal practice scenario using Two acceptable choices, an implementation note, a chosen improvement outcome and a separate follow-up note.
- The complete scenario report retained all seven timeline events and one recorded intervention attempt. Fictional training labels and hypothetical-versus-recorded distinctions remained visible.
- Branches and Possible Outcomes, Future Intervention Guide and Complete Training Packet previews rendered. The packet included all three report sections.
- Activated Print and Save as PDF; document title matched PolarisTrainingReportLopaze11-01-26through11-01-26. The cloud browser did not expose an actual saved print PDF.
- Downloaded the session JSON through the UI and parsed it: trainingOnly true, complete true, one attempt, seven events, fictional notes retained.
- Saved Records displayed one completed fictional record. Reload returned to sign-in, so reopen-after-reload and session continuity remain unresolved.
- No email sent; no real cadet information entered; no Supernova vault created or changed. The clearly fictional Polaris test record remains available in this browser storage.
T026 iPad workflow and navigation review
User confirmation and authenticated browser navigation
- User confirmed iPad PDF creation, saving and print-to-PDF work. Intended printing is from the emailed/saved PDF on a desktop or Chromebook; direct iPad printer output is not required.
- After secure sign-in, shared account displayed active access for all three applications. Nebula opened with the shared observer identity.
- Nebula menu opened Supernova with the shared observer identity. Supernova showed its separate device-vault setup; no vault or records were created or changed.
- Polaris opened under the shared account and presented its required Terms of Use and hold-harmless agreement. Scenario and report testing paused for user review/acceptance; no agreement accepted by the agent.
- Returning from the Supernova menu to Account and apps displayed sign-in again. Session continuity on return needs further investigation; this is not marked passed.
- No email sent; no credentials or real cadet data recorded in this log.
T025 Authenticated report action checks
Fictional Nebula observations in the signed-in COSMOS account
- Shared account profile populated the observer as Sam Lopaze. Three activations recorded one Participating entry.
- The bottom Save PDF button downloaded a real two-page landscape PDF. Its document title matched NebulaObservationLopaze11-01-26through11-01-26.
- Found that the bottom Email action did not expand the recipient section. Fixed in R028, reloaded and retested: the address section opened and its input received focus.
- Used qa@example.invalid to open the address confirmation dialog, then chose Go back and cleared the test address. No email sent.
- The bottom Print control was activated, but this cloud browser did not expose a print-preview window. Physical print output remains unverified.
T024 Report action verification
Deployed fictional UI, isolated generation checks and hosting validation
- Live fictional observation showed Could not observe, distinct from None Observed. DOM bounds placed feedback 28 pixels below the behavior grid without overlap.
- Review screen showed Print report, Save PDF and Email report below the signature certification. Download payloads and email confirmation handlers are reused.
- Filename tests passed in UTC and America/Los_Angeles: sorted ranges, invalid-date rejection, nested training sessions, safe surname, and multipart extensions. Zoned timestamps use the local calendar.
- Actual isolated PDF generation passed for normal two-page and long-note three-page reports. Email attachment name is validated by the server; PHP syntax and Supernova/Polaris inline CSP hashes passed.
- No email was sent and physical printing was not tested. Browser automation became intermittent before a new download could be completed through the bottom action; generated PDF and filename functions passed separately.
T023 Report usability and filenames
User feedback and implementation checks
- User reported that the Nebula observation and editable report worked successfully on an iPad.
- Filename helper checked for date ordering, invalid dates, safe observer surname and descriptive document extensions.
- Download payload structures, encryption settings and incident records are unchanged. Inline script CSP hashes updated without weakening the policy.
- Bottom-action UI and generated filenames require a fresh observation after deployment; physical printing and sending an email were not performed by this check.
T022 Nebula live report workflow
Temporary fictional browser fixture using deployed Nebula scripts; email disabled
- Started a one-minute observation, recorded one Task Refusal entry through three activations, ended early and opened review.
- Typed separate minute and entry notes. Both remained after editing a general outcome field and rerendering the report.
- Downloaded actual editable HTML, Excel XML and PDF through the live UI. HTML and XML retained both note texts; XML parsed successfully.
- Rendered and inspected both pages of the actual downloaded PDF: notes, landscape grid, reviewer and signature/date footers present.
- Authenticated shared-profile integration, editing and resaving a reopened offline report, physical printing and Excel application rendering remain pending. No test email sent.
T021 Nebula report grids
Isolated report-function tests and actual generated PDF inspection
- Minute and entry rows, editable note cells, safe text escaping, missing-minute distinction, two worksheet export and unsigned reviewer fields passed.
- Generated a real two-page landscape PDF and inspected both pages; widened minute columns after inspection. Long notes produced a three-page PDF without truncating the note array.
- Excel XML parsed successfully; formula-like notes exported as text. Observation core and original app scripts preserved byte-for-byte on install.
- Live editing, downloaded editable copy, physical printing and spreadsheet-application rendering remain separate checks.
T020 User device checks and save guidance
User-reported phone and iPad checks; hosting source validation
- User confirmed readable phone controls and successful iPad access, readable layout and photo presentation.
- User identified that an unselected cadet code prevented saving; selecting the code resolved the issue.
- Added a visible cadet-code reminder while preserving inline scripts and save validation byte-for-byte.
- Automatic PDF filename and large-text zoom checks remain pending.
T019 Shared sign-in consistency
Source validation and public entry-page check
- Credential forms and Supernova inline scripts preserved byte-for-byte. No account database, password, role, session-cookie or encryption change.
- Public account links consolidated on the branded sign-in entry. Existing central account service continues to handle all three apps.
- Account page describes the shared login; device-vault prompt explicitly distinguished. End-to-end sign-in still requires private user authentication.
T018 Supernova header update
Hosting source checks
- Header artwork replaced with the COSMOS-style orbit mark and responsive wordmark. Legacy banner removed.
- Inline app scripts are byte-for-byte unchanged; vault-lock and device elements retained. No CSP change needed.
- Shared header CSS published in both locations with a fresh cache version. Authenticated app visual and physical-device review remain pending.
T017 Readability and filename deployment
Live hosting file validation, local filename tests, and desktop browser measurements of a fictional layout fixture
- Responsive assets installed in both shared locations; gateway cache version updated. Report title uses the generated filename and the inline script matches the security policy hash.
- Filename tests passed for chronological date range, same-day range, missing profile/date fallbacks and safe filename characters.
- Eight behavior/severity color palettes have calculated contrast ratios from 6.57:1 to 11.05:1.
- At a desktop viewport of 1363 pixels, all 12 fictional behavior/severity buttons measured 18-19px text, at least 59px tall, with no horizontally clipped button content or page overflow.
- Real account session had expired, so updated authenticated incident workflow and actual filename suggestion in a new Save dialog were not rerun. Phone/tablet rendering, large-text zoom and physical-device tests remain pending. Embedded viewport checks were blocked by the existing framing policy; that protection was preserved.
- Earlier pending entries remain historical snapshots; T016 records the completed desktop restore and actual uploaded PDF inspection.
T016 Desktop recovery and exported PDF
User desktop screenshots, reported photo previews and uploaded PDF review
- First restore added two fictional incidents; second preview identified zero additions and two existing records; final Logs contained exactly two events.
- Both follow-up contents were visible and the user confirmed both differently colored test photos opened.
- Uploaded six-page PDF visually contained both incidents, both follow-ups, two distinct photo pages and unsigned reviewer section. Some pages have substantial whitespace.
- Responsive style checks cover syntax, target sizes and calculated color contrast only. Live responsive rendering and physical-phone checks remain pending.
T015 Live backup verification and screen recheck
Confirmed results from the real cloud-browser session; optional receipt download was interrupted by a browser-control timeout
5 passed checks
- Live read-only backup check passed without another passphrase prompt: two incidents, two photos, two unique incident IDs, two identical existing IDs and no conflicts or potential additions. No records were imported, overwritten or modified.
- Updated Logs view hides the guided incident editor.
- Fictional test incident remained present once with two follow-ups after reload and private unlock.
- Recovery menu computed styles confirmed light text on a dark background.
- Recovery panel opened without obsolete status text on the updated app load.
Pending checks & limits
- Optional verification-receipt saving was not confirmed because the cloud-browser connection timed out after the successful check.
- Recovery-panel reopening after a nonempty status message still needs a live recheck.
- Actual saved-PDF inspection and physical-device restore remain pending. Earlier isolated recovery tests are not a physical-device test.
T014 Same-vault backup verification
Deployed-code regression checks in isolated contexts; live check awaits reload
2 test suites passed
- Combined incident, follow-up, report and recovery regression suite passed after the verification change.
- Matching backup verified using the active vault key without a passphrase prompt or record changes; a different key still requested a passphrase, and cancel left records unchanged.
Pending checks & limits
- Live verification and the earlier visual fixes require the updated app to be reloaded and unlocked privately.
- Actual PDF and physical-device recovery checks remain pending.
T013 Live interface issues and fixes
Issues observed in the live browser; fixes installed for the next page load
Pending checks & limits
- Recovery-menu button contrast: fixed in shared CSS; visual recheck pending reload.
- Guided incident editor appeared above Logs: fixed with a scoped Logs-view rule; visual recheck pending reload.
- Recovery panel carried an old status message: opening it now clears the status; live recheck pending reload.
T012 Live incident, follow-ups, report and backup
Real cloud-browser checks with one separate fictional incident and two synthetic images
10 passed checks
- Saved a fictional incident with two photos; encrypted-save confirmation appeared.
- Added two follow-ups through the live form.
- Optional recurrence and next-steps fields persisted.
- Original response and outcome remained unchanged.
- Saved log displayed both follow-ups in chronological order.
- Saved numbered photo preview loaded within the screen.
- Actual generated browser report contained both follow-ups and both photo pages; both images decoded.
- Actual encrypted backup downloaded and the app confirmed its record count.
- Downloaded backup used the supported encrypted envelope; test code and narrative were absent from plaintext.
- Empty follow-up was rejected; closing the form retained exactly two saved follow-ups.
Pending checks & limits
- An actual saved PDF still needs inspection.
- Live backup decryption/verification needs the original vault passphrase entered privately.
- Physical-device restore, duplicate handling and layout on a phone remain pending; simulated recovery tests already passed.
T011 Live account and app entry
Real cloud-browser checks after private user sign-in and vault unlock
3 passed checks
- Account sign-in completed and all three apps showed active access.
- Supernova opened behind the account access gate.
- Recovery and follow-up panels initialized; vault remained locked until the user unlocked it privately.
T010 Account recovery prerequisites
Owner-authorized read-only account diagnosis; no password values inspected or modified
3 passed checks
- Account verification and enabled status confirmed.
- Completed password-reset event confirmed.
- No active sign-in lockout found.
Pending checks & limits
- The latest secure sign-in attempt still returned Invalid credentials. Direct user sign-in remains required before interactive app tests.
T009 Publication, access and sign-in checks
Current deployed-state and browser checks
4 passed checks
- Downloaded deployed source matched the source used for the isolated tests.
- Application inline-script Content Security Policy hash matched its deployed source.
- An anonymous application request redirected to account sign-in.
- Development-page Checks & tests text link opened the test history in the browser.
Pending checks & limits
- Live account sign-in returned Invalid credentials. Interactive app checks remain blocked; no automatic retries were made.
T008Combined incident → follow-up → report → recovery
Current isolated checks of the deployed application code; two simulated device vaults
16 passed checks
- Create a fictional incident with two photos
- Append two follow-ups to the saved incident
- Original observation, observer and photos remain unchanged
- Render report with chronological follow-ups, escaped text and two photo pages (print window simulated)
- Generate verified encrypted backup through deployed download handler
- Check backup using deployed read-only verification handler
- Recovery rehearsal preserves encrypted database without writes
- Restore backup into a separate simulated device vault; photos and follow-ups retained
- Repeated restore skips identical incident and preserves destination vault key
- Wrong backup passphrase fails without modifying destination records
- Different follow-up timelines stop restore as a conflict; neither is overwritten
- Guided incident correction retains existing follow-ups
- Failed encrypted write rolls back follow-up and audit changes
- Tampered encrypted backup rejected without writes
- Locked vault rejects follow-up saving
- Follow-up form identifiers unique; optional fields and secondary actions collapsed by design (source check)
Pending checks & limits
- Live browser checks blocked: the sign-in attempt returned Invalid credentials. No automatic retry was made.
- Report HTML and print invocation checked with a simulated print window; actual saved PDF not inspected in this run.
- Two isolated JavaScript device-vault contexts used; no physical-phone or cross-device browser restore performed.
T007Follow-up regression suite
Isolated regression checks rerun against the deployed code
4 passed checks
- Append-only save retains original fields, photos and observer.
- Encrypted round trip and single-incident transfer normalization retain follow-ups.
- Duplicate/conflict comparison, input validation and locked-vault denial checked.
- Failed storage writes roll back changes; incident corrections retain timelines.
Pending checks & limits
- This suite does not exercise live browser controls.
T006Shared profile and focused screens
Earlier integration/browser checks and user-reported phone feedback; not rerun here
4 passed checks
- Required profile gate, invalid-input rejection, safe return target and permission handling checked.

- That phone feedback preceded the backup panel and follow-up workflow updates.
T005Compact photo attachments
Earlier isolated browser fixture checks; not rerun here
3 passed checks
- Large fictional photos stayed within a narrow phone-width form.
- Numbered previews opened/closed; removal and renumbering worked.
- Photo data and printed-photo structure preserved by the patch.
Pending checks & limits
- A physical phone and all saved-photo PDF combinations were not checked in that fixture.
T004Aggregate visitor counts
Earlier recorded privacy, access and browser checks; not rerun here
3 passed checks
- Owner report access and anonymous denial checked.
- Allowed page categories counted in the browser.
- Analytics payload/storage checked for aggregate-only design, with no visitor identifiers or analytics cookies.
