Create an incident report

Document the event, response, and outcome as separate facts.

All guides · table of contents
  1. Start an event

Open the vault using your own passphrase. Choose Record / Record an event and select a fictional code such as DA; never type a cadet name during practice.

  2. Identify the event

Select the observable behavior and severity using the app controls. Record date/time, setting, activity, and observer where requested. Use Next and Back to review the six steps.

  3. A: antecedent

Describe what happened immediately before the event. Fictional example: Staff presented a two-item practice task. Do not state an assumed motive as a fact.

  4. B: behavior

Record what was seen or heard. Example: Demo A pushed the practice worksheet away twice within one minute. Add duration or frequency when actually measured.

  5. C: consequence and response

Describe what followed and the strategy actually attempted. Example: Staff offered a choice of which item to begin. Consequence means what followed, not necessarily punishment.

  6. Outcome and attachments

Describe the observed effect and recurrence separately. Add only relevant, permitted photos or notes. A recommendation alone is not an attempted response.

  7. Review and save

Check the event, code, timeline, and facts. Choose Save event and verify the encrypted-save confirmation. For actual emergencies, use applicable safety procedures; Supernova is documentation support.
